The CCP Staff

The Center for Community Partnerships (CCP) is a direct service and outreach arm of the Department of Occupational Therapy at Colorado State University. In operation since 1985, CCP is a fee-for-service program that provides comprehensive and individualized services for youth and adults with disabilities and/or challenges as they pursue employment, independent living, recreational and community access goals. In addition, consultative and educational services are provided for K-12 schools and colleges/universities in regards to accommodations and supports for students with disabilities as they pursue their educational goals.

Specific services available through the CCP include the following:

  • Supported employment services, including long-term support, for youth and adults with developmental disabilities, mental illness, brain injury or other disabilities.
  • Career planning and job development services, identifying the ideal job match for individuals with disabilities.
  • Home and community-based occupational therapy services for children, youth and adults, identifying and implementing adaptations that will promote development and independence.
  • Occupational therapy ergonomic evaluation and support.
  • Consultation with children with disabilities, including brain injury, their families and K-12 educators.
  • Individualized support and mentoring for college students with disabilities, providing assistance with personal adjustment to college life, study and organizational skills, and connection to college resources.
  • Consultation and educational services around best practices in higher education through the principles of universal design for learning.
  • Consultation and educational services around the Americans with Disabilities Act (ADA) for employers and educators.

Individuals may be referred to CCP for services via the Colorado Division of Vocational Rehabilitation, the Colorado Division for Developmental Disabilities (Foothills Gateway, Inc.), the Colorado Workforce Center, Veterans Affairs, Larimer County Department of Human Services, Colorado TBI Trust Fund, grants and school systems, as well as through self-referral/self-pay.
