A Program for Community Skills Development
Supported Employment Services for Adults with Disabilities

The Transition Services Program is staffed with highly qualified professionals with degrees in occupational therapy and psychology, and over 40 combined years of experience working with individuals with the most challenging of disabilities. The CCP Transition Services staff are experts in the field in the area of supported employment services for adults with challenging disabilities, including but not limited to adults with developmental disabilities and mental illness. Individualized, community-based and integrated supported employment services include the following:
- Functional Community-Based Assessment: Transition Services staff work closely with referred participants, providing opportunities for them to try out jobs at employment sites in the community to determine strengths, abilities and support needs, as well as to establish whether the participant is interested in and qualified for different types of employment. For participants who are unable to drive, Transition Services staff will also determine, through observation in the community, what supports the participant may need to utilize public transportation and fully access the community. The assessment process may take 4 – 6 weeks, in that all assessment activities are at multiple sites the community.
- Job Development: Following the assessment process, participants will work with a Transition Services staff member and a CCP Job Developer, developing a resume and applying for jobs in the community that match identified strengths, abilities and support needs. Transition Services staff take pride in identifying and securing jobs that uniquely match each participant’s identified skills and abilities, targeting employment that the participant is qualified for and insuring that the employer’s needs are met.
- Employment Consultation: Once the participant has been hired, Transition Services staff will work with the participant and the employer, insuring that supports are in place for training and completion of essential job functions. Transition Services staff will identify any adaptations or accommodations that are needed at or away from work that will promote employment success. In addition, Transition Services staff will assist with on-the-job training, as needed, and will provide job coaching to insure the participant understands and is able to fulfill all aspects of the job. If requested or needed, CCP staff will provide disability sensitivity support and training for co-workers and the employer, promoting natural supports in the work environment.
- On-going Support: For adults with developmental disabilities, on-going follow-along support will be provided even after the participant is fully trained at his/her job, to insure continued employment success. CCP’s Transition Services staff will work closely with each participant’s employer, identifying natural supports in the work environment and insuring that encouragement and support is always available at the worksite. In addition, Transition Services staff will maintain close communication with family members, guardians and professionals from other agencies who are supporting the participant, maintaining collaboration and partnerships that promote success for each participant.
